[Lapack] extra precision refinement routines

Dear lapack list,

I'm hoping someone can clear something up about some of the arguments
for the new routines DGERFSX, DPORFSX, DSYRFSX, DGBRFSX, ZGERFSX,
ZPORFSX, ZSYRFSX and ZGBRFSX.

Each of them has either 1 or 2 arguments (S or R and C) that are
labelled "input or output" with the description mentioning the character
FACT being used to determine which it is. For example: "S is an input
argument if FACT ='F'; otherwise, S is an output argument." However none
of these subroutines have FACT as an argument.
